CNIC Loss Affidavit

When your National Identity Card (CNIC) is lost or stolen, NADRA requires a sworn affidavit before issuing a duplicate card. This document formally declares the circumstances of the loss and absolves you of liability for any misuse of the original card.

§When You Need This Document

• •Lost CNIC application at NADRA
• •Stolen CNIC — police FIR copy also recommended
• •Damaged CNIC that cannot be used
• •Required by banks, employers or government offices

§Stamp Paper Requirement

PKR 50 stamp paper is typically accepted. Confirm with your local NADRA office.

AFFIDAVIT FOR LOSS OF CNIC

   That I am a Pakistani national and the holder of CNIC No. [CNIC Number], issued by NADRA.

   That despite thorough search, the said CNIC has not been found and its whereabouts are unknown.

   That I have not sold, pledged, transferred or handed over my CNIC to any person, and I am not responsible for any misuse thereof.

   That I require a duplicate CNIC from NADRA and this affidavit is being made for that purpose.

   That if the original CNIC is found in future, I shall surrender the same to NADRA immediately.
